What Is DA Form 705-TEST?
DA Form 705-TEST is the Army Fitness Test Scorecard, prescribed by ATP 7-22.01 (Holistic Health and Fitness Test Protocols, published 12 March 2026). It replaced DA Form 705, the scorecard used for the retired Army Physical Fitness Test (APFT), and is the single official form used to record Army Fitness Test results.
When you complete an AFT event, the grader records the number of correct repetitions, the distance, or the time on the DA Form 705-TEST and initials the scorecard. After your 2-Mile Run time is entered, the event grader converts the raw scores for all five events into scaled scores using the official AFT scoring tables, enters the total on the form, and the OIC/NCOIC confirms the graders have properly initialed each event.
The form also records your demographic information and all event scores. Height and weight are not recorded as event scores on DA Form 705-TEST; body-composition requirements are handled separately under current Army policy.
DA Form 705-TEST Fields, Explained
The scorecard has two sides. The front captures personal data and the five event raw scores; the back carries the scoring scale reference. What each field means:

Each event on the AFT is scored 0 to 100 points, so the maximum AFT total is 500. AFT scoring requires at least 60 points in each event. For combat specialties, the current standard is at least 60 points per event and 350 points overall; overall passing requirements for other Soldiers depend on the applicable Army standard and Soldier category. Any event below 60 points fails the test regardless of the total.
How the DA Form 705-TEST Is Filled In
The scorecard moves through a fixed sequence on test day, from pre-test data entry to the final signature. Understanding the flow shows what to expect and what to check before you sign. Test-day rules, including what happens after a failure, are on the test day and retest page.
- Pre-test: personal data: The OIC or NCOIC hands one DA Form 705-TEST to each Soldier. In the appropriate spaces, print in ink the required personal information. Event supervisors read the instructions aloud to all Soldiers.
- During each event: raw scores + initials: As you complete each event, the grader records the raw result — reps, distance, weight, or time — in the raw score block and initials the scorecard. On the SDC, two graders work together to measure distance and time.
- After the 2-Mile Run: conversion: Once your 2-Mile Run time is entered, the event grader converts every event's raw score into points using the official AFT scoring scale and enters the total, while the OIC/NCOIC oversees the process and confirms all graders have initialed.
- Final step: your signature: The grader confirms the scores with you, and you sign the DA Form 705-TEST to verify accuracy. The grader then returns all signed test forms to the OIC or NCOIC.
Who Fills In the Scorecard: Grader, OIC, NCOIC
The DA Form 705-TEST is a team effort. Graders administer individual events and record raw results; the NCOIC manages the overall test; the OIC supervises it. Each role has specific scorecard responsibilities under ATP 7-22.01.
Graders record the correct number of repetitions, distances, weight, and times in the raw score block on the DA Form 705-TEST, convert raw scores to scaled scores, address and resolve scoring discrepancies, and record their initials and signatures. The OIC supervises the AFT and the NCOIC manages the test, which includes keeping a running clock and overseeing score conversion after the final event. How the Scorecard Connects to Your AFT Score
The DA Form 705-TEST holds the raw numbers, but your pass/fail status comes from the scoring scale. Each AFT event is scored 0-100 points. AFT scoring requires at least 60 points in each event. For alternate aerobic events (2.5-Mile Walk, 12,000-Meter Bike, 1,000-Meter Swim, 5,000-Meter Row), the grader records the time on the DA Form 705-TEST and annotates go or no-go. DA Form 705-TEST vs the Old DA Form 705
The AFT introduced a new scorecard because the test itself changed. The old DA Form 705 recorded the APFT — push-ups, sit-ups, and the two-mile run. The current DA Form 705-TEST records the five AFT events: the deadlift, hand-release push-up, sprint-drag-carry, plank, and two-mile run.
